Self-care in New Zealand: patient perspective survey Print

Are you aged 18 and over and live with arthritis and/or other long-term conditions? (e.g. asthma, diabetes, bipolar disorder, emphysema, COPD, other lung disease, heart disease or other long-term conditions.)

Mandy is a PhD student with the Centre for Postgraduate Nursing Studies and this survey forms part of her research project.

The aim of the survey is to obtain a picture of what it is like to live with a long-term condition (e.g.: asthma, arthritis, diabetes, bipolar disorder, emphysema, COPD, other lung disease, heart disease or other long-term conditions ) in New Zealand.

Mandy is seeking adults, aged 18 and over, living with a long-term condition to participate in this survey. Your participation in this survey is entirely voluntary (your choice) and the online survey takes approximately 20 minutes to complete. The survey has received ethical approval from the Multi-region Ethics Committee, New Zealand.

The answers from all survey participants will be gathered together and analysed. Results from the survey may be published and no material that could personally identify you will be used in any reports about this survey. The data from the survey will be retained in secure storage for ten years, after which it will be destroyed.
